Granite Bay Care, Inc. (GBC) is a locally owned and operated human service agency with strong connections to the central and southern Maine community. GBC works closely with the Department of Health and Human Services office in each Region to accommodate the multi-faceted needs of each supported consumer as an "individual" with unique hopes and goals. The GBC Service Team meets as needed with the representatives from the referring region to ensure that all services are provided in a manner that is efficient and fully responsive to the personal needs of each individual. GBC staff work closely with other participants in each individual's circle of support, especially family members.
The framework for all service provisions is the Person Centered Plan (PCP). Through the PCP, programming is developed to support the individual at their highest level of functioning. While offered supports may vary according to the specific needs of the individual, from assistance with medication administration, daily living skills, to community integration, all services are rendered in residential homes. GBC develops and maintains single gender residences; these one or two person homes are situated in rural settings that balance privacy with access to community supports and infrastructure.
Securing and maintaining paid employment, participating in activities of daily living, and enjoying local opportunities for recreation, all individuals are encouraged to become active members of the communities within which they reside. GBC support persons with a variety of diagnoses ranging from Clinical Syndromes (i.e. psychosis, bipolar disorder, major depressive disorder, obsessive-compulsive disorder, etc) to Personality and Developmental Disorders (i.e. antisocial personality disorder, post traumatic stress disorder, borderline personality disorder, pervasive developmental disorder, mental retardation, etc). GBC is committed to supporting individuals with challenging behaviors, even those with extensive involvement within the criminal justice system. |
